How to Make a Claim
PERSONAL ACCIDENT
If you are a member of AusTriathlon and you have suffered an injury whilst participating in an official AusTriathlon activity (including training, racing, competing or other sanctioned activities) please follow the below steps to lodge a Personal Accident claim.
Please refer to the Policy Wording for policy terms, conditions and exclusions

Step 2:
Complete each section of the claim form. Please be advised that incomplete claim forms may cause a delay in processing your claim.
Step 3:
Please return your Personal Accident claim form and supporting documentation to DWF Claims Management via email, post or fax:

Step 4:
DWF Claims Management will confirm receipt of your claim form and advise whether they require further information.

Important information
The Health Insurance Act (Cth) 1973 does not permit the insurer to contribute to any charges covered, or partially covered by Medicare. Please refer to the table below for some common examples:

| *MRI scans are generally covered through Medicare; however please check with your treating physician, as sometimes the provider is not registered with Medicare. |
In addition, there will be no refund for any expenses to which the National Health Act (Cth) 1953 applies.
TRAVEL INSURANCE
If you need emergency assistance while travelling, please refer to the emergency contact details detailed on the Travel Summary of Cover Brochure and policy wording provided when upgrading membership to include travel insurance.
You can download an AusTriathlon Travel Claim form through the link below. Before lodging this form, please ensure all sections are fully completed as truthfully and accurately as possible. Failure to complete all sections of this form properly may delay settlement of your claim.
Only one claim form (per journey) is required. A claim form should be completed and submitted as soon as you become aware that you will be making a claim.
Once you have completed your claim form, please forward with all relating documentation to

PUBLIC LIABILITY
If you are an AusTriathlon Member, Affiliated Club or Approved Event Manager, please complete this Incident Report Form if;
- Someone is holding you responsible for damaging their property or caused injury.
- An incident has occurred and you have damaged someone’s property or caused injury.
