How to Make a Claim

PERSONAL ACCIDENT
If you are a member of a Squash Australia Club or Centre and you have suffered an injury after 30 March 2023 and whilst participating in a squash or squash related activity (including training, competing, official social event, fundraising activity or other sanctioned activity) please follow the steps below to lodge a Personal Accident claim. If your injury occurred before 30 March 2023, please call V-Insurance Group on 02 8599 8660 or 1300 945 547.

STEP 1 Download a Squash Australia claim form.

Personal Accident Claim Form

STEP 2 Complete each section of the claim form. The declaration by your Club/Centre must be completed by a nominated official. Incomplete claim forms may cause a delay in processing your claim.

STEP 3 Please return your Personal Accident claim form and supporting documentation via email, post or fax to the Personal Accident claims handler, Corporate Services Network who will process your claim on behalf of Liberty International Underwriters:

STEP 4 Corporate Services Network will confirm receipt of your claim form and advise whether they require further information.

Important Information
The Health Insurance Act (Cth) 1973 does not permit the insurer to contribute to any charges covered, or partially covered by Medicare. Please refer to the table below for some common examples:

*MRI Scans are generally covered through Medicare; however please check with your treating physician, as sometimes the provider is not registered with Medicare.

In addition, there will be no refund in respect of any expense to which the National Health Act (Cth) 1953 or any of the regulations made there under apply.

Public Liability, Products Liability, Professional Indemnity and Association Liability

If an incident occurs where you are being held liable by a third party you should contact V-Insurance Group as soon as possible on (02) 8599 8660 or local call cost only 1300 945 547. Do not admit liability under any circumstances.
