How to make a Claim
PERSONAL ACCIDENT
Step 1: If you are an eligible member of Bicycle Queensland and you have suffered an injury whilst riding your bike or participating in an official Bicycle Queensland activity, please contact Bicycle Queensland for a claim form.

Step 2: Your completed claim form and supporting documentation should be submitted directly to DWF Claims Management for assessment and processing. DWF Claims Management is the current claims handling service representing the insurer for the Personal Accident program, Chase Insurance.
Step 3: DWF Claims Management will confirm receipt of your claim and advise whether they require further information. They can be contacted by phone on (02) 9287 1302 or email [email protected] if you need an update on the status of your claim.
Important information
The Health Insurance Act (Cth) 1973 does not permit the insurer to contribute to any charges covered, or partially covered by Medicare.

| *MRI scans are generally covered through Medicare; however please check with your treating physician, as sometimes the provider is not registered with Medicare. |
In addition, there will be no refund for any expenses to which the National Health Act (Cth) 1953 applies.
PUBLIC LIABILITY
If you are a Bicycle Queensland member or a Bicycle User Group (BUG) member, and someone is holding you responsible for property damage or personal injury arising from an incident, please contact Bicycle Queensland as soon as possible on +61 (7) 3844 1144.
