PERSONAL ACCIDENT
If you are a member of Australian Outrigger Canoe Racing Association (AOCRA) and you have suffered an injury whilst particpating in an official AOCRA activity (including training, competing, or other sanctioned activities) please follow the below steps to lodge a Personal Accident claim.
Please refer to the Policy Wording for policy terms, conditions and exclusions.
Step 1:
Download a Australian Outrigger Canoe Racing Association (AOCRA) Personal Accident Claim Form:
Step 2: Complete each section of the claim form. Your affiliation must be verifies by both a club and state official. Please be advised that incomplete claim forms may cause a delay in processing your claim.
Step 3: Please return your Personal Accident claim form and supporting documentation to Corporate Services Network via email, post or fax:

Step 4: Corporate Services Network will confirm receipt of your claim form and advise whether they require further information.

Important information
The Health Insurance Act (Cth) 1973 does not permit the insurer to contribute to any charges covered, or partially covered by Medicare.

In addition, there will be no refund for any expenses to which the National Health Act (Cth) 1953 applies.

CYBER LIABILITY & PRIVACY PROTECTION
As soon as you become aware of a potential Cyber Liability claim or a breach of Privacy, you must notify the insurer Chubb Insurance. Chubb have an incident hotline in place to help you manage matters. This incident hotline is handled by the International Service provider, they will help you with finding a resolution to your matter.

MARINE HULL INSURANCE
If your marine canoe craft: outrigger, ski, boat vessel has been damaged or stolen, please ensure that you follow these steps;
- Do what you can to prevent any further loss, damage, cost or liability.
- Report the damage, theft, malicious act or vandalism to police and obtain a police report number.
- When safe to do so, take photos of the damage.
- Obtain quotes for repair or replacement.
- Download and complete a claim form.

CLUB CANOE TRAILER
If your insured trailer has been damaged or stolen, please follow these steps;
- Do what you can to prevent any further loss, damage, cost or liability.
- Report the damage, theft, malicious act or vandalism to police and obtain a police report number.
- When safe to do so, take photos of the damage.

We will send you a claim form for completion and will likely request you obtain quotes for repair or replacement.
In the event of large losses, a loss adjustor/investigator may be appointed by the insurer to assist with the claim.
